How to Refer Patients

Appointments require a referral from a dental or medical provider

  • LSU School of Dentistry: Referral by dentist or physician only: 
    • Appointments in the dental school clinic are made by referral from dentist or physician.  Doctors may fax a referral letter to OMS, indicating the patient's name, contact number, and the specific reason for the referral.  The patient will receive a call from the OMS scheduling clerk within 3-4 weeks.  Doctors should call 504-941-8408 or fax to 504-941-8164 to refer a patient.
  • LSU Oral and Maxillofacial Surgery Faculty Practice (New Orleans & Baton Rouge Locations): Referral by dentist or physician only
    • Patients are accepted upon referral from their dental or medical provider.  Please call 504-889-9893, fax 504-941-8490 or email at uomfs@lsuhsc.edu to refer a patient or schedule an appointment for our New Orleans office. Please call 225-769-2774 for our Baton Rouge office.

  • University Medical Center: Self-referral Clinic:
    • To schedule an appointment, call 504-702-5125.  Dentists or Physicians can fax referrals to 504-702-5742. Appointments are required.
  • Our Lady of the Lake (LSU Health Perkins Surgery Center: 9032 Perkins Road, Baton Rouge, LA 70810):
    • ​Appointments can be made by referral only.  Fax referral to 225-354-2075.  
  • LSU Faculty Practice (New Orleans & Baton Rouge Locations):
    • ​Patients are accepted upon referral from their dental or medical provider.  Please call 504-889-9893 to refer a patient or schedule an appointment for both our New Orleans and Baton Rouge offices.  

 

Treatment Fees

  • LSU School of Dentistry:
    • ​For limited care Oral and Maxillofacial Surgery, there is a $50 consult fee and $70 for panorex x-ray.  Only panorex x-rays taken within one year will be accepted from outside referrals and can be emailed to pcorte@lsuhsc.edu.  Please be sure to include the date and patient's name on the x-ray prior to e-mailing or include this information in the e-mail.  
    • The cost for an extraction ranges between $102 and $294, which will be determined at the time of the consult.  
    • Treatment in other departments at the LSU School of Dentistry requires additional fees. Referrals involving multiple extractions, implants or comprehensive treatment will be referred to the adult screening department.  
  • University Medical Center:
    • Oral and Maxillofacial Surgery fees are on a sliding scale, based on the patients's ability to pay.  
  • LSU Faculty Practice (New Orleans & Baton Rouge Locations):
    • Most major medical and dental plans are accepted.
