Cancellation & Refund Policy:
The registration fee is partially refundable (minus an administrative fee) if you cancel your registration thirty (30) days before the start of the course. Any cancellation after this deadline will result in forfeiture of the entire tuition. Written cancellation requests can be submitted by email to CDE@lsuhsc.edu. LSU Health New Orleans Continuing Dental Education (LSU CDE) reserves the right to cancel the course should circumstances warrant such action. Full refunds of registration fees would be made in that event. However, LSU CDE is not responsible for any related costs or expenses, including cancellation/change charges assessed by airlines.
Health & Wellness (Physical Attendance) Policy:
By registering for an in-person course, you affirm that you will comply with the safety requirements of the course facility/location (if only) with regards to wearing face masks, getting temperature taken, etc. You also affirm that you will NOT attend the course if you have a fever, respiratory/flu-like symptoms and/or if you believe that you have been exposed to COVID-19. If you are ill and/or feel that you should self-quarantine and will not be able to attend the course (in-person), contact our office (504.941.8193 or CDE@lsuhsc.edu). It is your responsibility to (i) ensure you are healthy enough to attend a course in person, (ii) abide by safety requirements of the course facility/location, and (iii) contact LSU CDE office (504-941-8193 or CDE@lsuhsc.edu) if you will be unable to attend a course due to illness. LSU CDE must be contacted PRIOR to the start date/time of the course. LSU CDE monitors updates from the CDC, as well as, local, state, and federal guidelines. If additional safety precautions become necessary, LSU CDE will inform registered attendees.
Nonrefundable Airline Ticket, Flight Change, or Hotel Reservation:
LSU CDE cannot be held responsible for the cost of a nonrefundable airline ticket, flight change, or hotel room reservation in the event a course is canceled or rescheduled. Please: No recording devices in lecture room or lab, and turn mobile phones off. New Knowledge Related to Dental Practice: You are cautioned about the potential risks of using limited knowledge when incorporating techniques and procedures into your practice, especially when the course has not provided you with supervised clinical experience to ensure you have attained competence.
New Knowledge Related to Dental Practice:
Dental educational institutions have an obligation to disseminate new knowledge related to dental practice. In so doing, some presentations may include controversial materials or commercial references. Participants are cautioned about the potential risks of using limited knowledge when incorporating techniques and procedures into their practices, especially when the course has not provided them with supervised clinical experience in the technique or procedure to ensure that they have attained competence. Providership or partial providership of a continuing education course by LSU Health New Orleans Continuing Dental Education does not necessarily imply endorsement of a particular philosophy, procedure or product.

Continuing Education Credit:
CE credit awarded for participation in a course or activity may not apply toward license renewal in all states. It is the responsibility of each participant to verify the requirements of his or her state licensing board. Credit counts are subject to change.

By attending this course/session, attendees agree to allow their names, likenesses and images either in audio, photographic or video format recorded on site to be used by LSU CDE for educational and promotional purposes.
