APPLICATION
INSTRUCTIONS - PROGRAM
IN DENTAL HYGIENE
Section I:
ADEA Application
- Apply to the dental
hygiene entry-level
program in DHCAS (https://www.adea.org/DHCASapp/)
- Pay appropriate ADEA DHCAS Fee for entry-level program
- Have official
transcripts mailed to:
ADEA DHCAS Transcript
Processing Center
P.O. Box 9129 Watertown,
MA 02471.
Separate Transcripts
must be submitted for
all colleges attended.
(Must submit request before
the last business day of December, 4:00 pm CST). If
necessary, have current
fall semester grades
submitted later but
before January 15th.
- Two letters of
reference, one from a
college professor and
the other from an
additional professor,
dentist, co-worker or
employer. These letters
are submitted
through DHCAS, a maximum
of three are accepted.
- Personal essay
- ATDH Score: Scores should be sent to both ADEA DHCAS and LSU Dental Hygiene
Section II:
Supplemental Application
- $50.00 Non-refundable
application fee paid online through the LSUHSC Fee Payment Portal. Your application
is not complete until all payments and required documents are received.
- Arrange to have the
following documents sent directly

- Resident
Verification

- Passport sized photo
(2 ½ x 2 ½) attached to
Resident Verification
form (will not be
returned)
- Copy of Driver's
License (front and back)
- Copy of your current
Draft Card status (males
only) available
at: www.sss.gov
- Official ACT scores
submitted directly to
our office from ACT.
Contact the ACT Office
at 319-337-1313 for
permission to release
scores to LSUHSC. The
ACT School code is 1601.
No photocopies accepted.
This requirement will
not be waived; placement
test will not substitute
for this requirement.
- Official ATDH
scores submitted
directly to our office.
No photocopies accepted.

Dental Hygiene applicants
must be residents of
Louisiana.
Your application will not
be processed until all of
the following
information/documentation
is received by the LSUSD
Office of Student
Admissions.
It is the
applicant's
responsibility to make
sure all items are
received by the
deadline, the last business day of December, 4:00 PM CST.
