LSUHSC transcripts may be
ordered and paid for online at www.getmytranscript.com.
Individuals who are unable to use the online
service may submit their requests by completing
this transcript request form and sending it to
our office, along with the appropriate payment
as indicated on the form.

Verification
Enrollment Verification
Certificates are available to currently enrolled
students. Students may print their own
verification certificate by using our secure Student Self-Service Portal
provided by the National Student
Clearinghouse. Outside agencies requiring
a verification of enrollment should
contact the Clearinghouse online at www.studentclearinghouse.org
or by phone at (703) 742-4200.
