Fill out the form below to register for your class reunion. Click on the CONTINUE button to be directed to the LSUHSC Foundation Donation page to make your payment.

Simply fill out your information, select OTHER and fill in the correct payment amount, and designate the School of Dentistry. In the COMMENTS section, make sure to notate your class year.

Refer to the dinner menus from your printed brochure and specify your and your guest's entrée choices: fish, beef or veal. Each class has different menu choices.
