Fill out the form below and click on the CONTINUE button to be directed to the LSUHSC Foundation Donation page to make your payment.

On the Foundation Donation page, simply fill out your contact information, fill in the correct payment amount and designate the School of Dentistry. In the COMMENTS section, make sure to notate GOLF CLASSIC. To download the Golf Classic brochure in a pdf file, click HERE.

If you are sponsoring a team, please enter the sponsor name exactly how it should be listed for recognition and publicity purposes.

If your sponsorship level allows for more than one team, please complete this form for each team; however, you will only pay once. Simply disregard when redirected to the LSUHSC Foundation after your initial contribution. Make sure you designate your sponsor name for each additional team. If you are registering for yourself ONLY, fill out the Player 1 information and we will place you on a team.

*Sponsorships provide varied amounts of complimentary spectator guests:
Platinum (4 guests), Gold (3 guests), Silver (2 guests) and Bronze (1 guest).

Note: Your registration is not complete until you make your  
payment on the Foundation Donation page.
