Application Process Instructions

APPLICATION INSTRUCTIONS - PROGRAM IN DENTAL HYGIENE

Section I:  ADEA Application

  1. Apply to the dental hygiene entry-level program in DHCAS (https://www.adea.org/DHCASapp/)

  2. Pay appropriate ADEA DHCAS Fee for entry-level program

  3. Have official transcripts mailed to:
    ADEA DHCAS Transcript Processing Center
    P.O. Box 9129 Watertown, MA 02471.
    Separate Transcripts must be submitted for all colleges attended. (Must submit request before the last business day of December, 4:00 pm CST). If necessary, have current fall semester grades submitted later but before January 15th.

  4. Two letters of reference, one from a college professor and the other from an additional professor, dentist, co-worker or employer. These letters are submitted through DHCAS, a maximum of three are accepted.

  5. Personal essay

  6. ATDH Score: Scores should be sent to both ADEA DHCAS and LSU Dental Hygiene

Section II:  Supplemental Application

  1. $50.00 Non-refundable application fee paid online through the LSUHSC Fee Payment Portal. Your application is not complete until all payments and required documents are received.

  2. Arrange to have the following documents sent directly to:
    LSU School of Dentistry, Office of Student Admissions
    1100 Florida Ave., Box 28
    New Orleans, LA 70119

  3. Resident Verification (Updated 11/2021)
    Requires PDF reader software to view it. You can download a free PDF Reader here.

  4. Passport sized photo (2 ½ x 2 ½) attached to Resident Verification form (will not be returned)

  5. Copy of Driver's License (front and back)

  6. Copy of your current Draft Card status (males only) available at:  www.sss.gov

  7. Official ACT scores submitted directly to our office from ACT. Contact the ACT Office at 319-337-1313 for permission to release scores to LSUHSC. The ACT School code is 1601. No photocopies accepted. This requirement will not be waived; placement test will not substitute for this requirement.

  8. Official ATDH scores submitted directly to our office. No photocopies accepted. See www.ada.org/ATDH for further details for submission of exam.

Dental Hygiene applicants must be residents of Louisiana.

Your application will not be processed until all of the following information/documentation is received by the LSUSD Office of Student Admissions.

It is the applicant's responsibility to make sure all items are received by the deadline, the last business day of December, 4:00 PM CST.
