LSU School of Dentistry Dental Laboratory
Technology's Applications are in PDF format and requires

The following forms, all required to complete your
application, can be downloaded from these links: Instructions Application Confidential Rating Residency Verification
In addition to your application, other materials must
be sent directly to us in order to complete your
application. Your application will not be processed
until all of the following information/documentation is
received by the LSUSD Office of Student Admissions.
The application deadline is June 30.
- Completed Application: Download
here All sections of the application must be
completed by the applicant.
- Application Fee: A non-refundable
fee of $50.00 (Subject to change without notice) must
accompany the application. Only checks will be
accepted. Make checks payable to
LSUHSC.
- Passport-Type Photograph: A
recent (2 ½ x 2 ½) color photograph (full face view)
must accompany the application, affixed to the upper
left hand corner.
- Photocopy of Driver’s License
- Official Transcripts; Official
transcripts from all colleges and
universities attended by the applicant must be sent to
the Office of Admissions. These materials should come
directly from the Registrar of the institution.
Transcripts in sealed envelopes signed across the seal
are also acceptable in special situations.
Hand-carried transcripts may be considered unofficial
and will not be accepted.
- Recommendations: Each applicant
is required to submit a confidential recommendation
form from one college professor who has instructed
them in a course. The confidential rating form should
be mailed directly to the Office of Admissions by the
teacher, since hand-carried forms delivered by the
applicant, by definition, have lost their
confidentiality.
- Residency Verification Download here.
- Copy of your current Draft Card status
(males only) you can print a copy off the internet
at www.sss.gov
It is the applicant’s responsibility to make
sure the following two items are sent directly to the
LSUSD Admissions Office and received by June
30:
1. Official transcripts sent by the registrar of
every college you have attended. It is your
responsibility to forward additional transcripts after
each semester is completed. 2. One
confidential rating form. Completed by a faculty member
who has instructed you in a college course. (Preferably
in a science course). Download here.

Although the DLT program application
deadline is June 30, be aware that federal
financial aid application priority processing
deadlines are as follows:
